The Butler County Board of Health conducted inspections of more than 60 food service establishments in Oxford since the Observer’s most recent issue. Here is a list of those in which violations were found:
A One Shop, located at 3604 Southpointe Parkway, was inspected July 18, and received two critical violations. Shop owners failed to store cleaning chemicals properly and failed to clean the smoothie machine as often as the Ohio Health Code requires. Both violations were corrected during inspection.
Oxford’s Burger King, located at 5093 College Corner Pike, received a follow-up inspection on July 18, and was met with four non-critical repeat violations. All four violations centered around the restaurant’s unsanitary food preparation with some violations citing grime buildup on the walls and floors of the kitchen. Additionally, inspectors mentioned the presence of soiled cardboard on the kitchen floors near the grills, making it harder for workers to clean the kitchen floor.
Colony Food Mart, located at 5284 College Corner Pike, received a follow-up inspection on July 19, and was given six non-critical violations. All six violations were relatively minor. Inspectors cited shoddy piping throughout the facility leading to leaks that left stains on the floor and ceiling tiles in one of the restrooms. Additionally, inspectors cited a lack of adequate cleaning facilities in the building, most notably a lack of a mop sink.
The in-house kitchen of McCullough Hyde Hospital, located at 110 N. Poplar St., was inspected on July 18, and received three critical and three non-critical violations. The critical violations included improper hand washing and glove use along with improper preparation of raw beef on an unsensitized surface. Along with this, McCullough Hyde was fined for improper use of their handwashing sink and a lack of adequate paper towels at that same sink.
Skyline Chili, located at 1 E. High St., was inspected on July 18 and received one non-critical violation. Inspectors cited cracked floor tiles in the back of the building near the front of the walk-in cooler. Inspectors recommend that the owners repair the tiles in the near future.
Snack Shack, located at 2560 Oxford Millville Road, received an inspection on July 17 and was given three non-critical violations. Inspectors cited the establishment for not having any employees with food safety training, the use of non-approved appliances for food production and an overall lack of facility upkeep.
Oxford’s Walmart, located at 5720 College Corner Pike was inspected on July 18 and received two critical and five non-critical violations. Critical violations were for a prevalence of dented or penetrated food storage containers on store shelves (mostly soups and other canned goods) along with improper storage temperatures for items near the hot deli section.
Circle K, located on College and High St., was inspected on July 25 and received one critical and two non-critical violations. No further details of the inspection were provided.
Fiesta Charra Mexican Restaurant, located at 25 W. High St., was inspected on July 25 and received zero critical and six non-critical violations. The non-critical violations include an improperly sealed backdoor, food grime on the non-food contact portion of grills, water stained ceiling tiles, two unused table top steam tables, lack of clean equipment, a cracked cove base in the kitchen, broken floor drain, and personal items of employees on the prep counter.
Moon Co-Op, located at 512 S Locust St., was inspected on July 25 and received one critical and three non-critical violations. Inspectors found the quaternary ammonium sanitizing solution at an incorrect balance, which is a critical violation. The non-critical violations include an improperly sealed backdoor, a residential crock pot, and an unused oven.
Bagel and Deli Shop, located at 119 E. High St., was inspected on July 27 and received zero critical and three non-critical violations. Inspectors found a residential microwave used in the facility to store and cook bacon only, damage to the reach-in cooler, and build up of food debris and grime on the doors of the reach-in coolers.
Papa John’s, located at 619 Locust St., was inspected on July 27 and received one critical and four non-critical violations. Inspectors found the air conditioner to be draining in the hand-washing sink. The non-critical violations include a manager without food protection certification, food grime on prep coolers and pizza ovens, a leak in the three compartment sink, a hole on the cooler door, cracked floor tiles, and employee personal belongings on the prep counter.
SDS Pizza, located at 7 E. Chestnut St., was inspected on July 27 and received zero critical and six non-critical violations. The non-critical violations include a manager without food protection certification, improperly named food containers, an ice machine near an unshielded sewer line, food grime on the walls near cooking equipment, food grime on the racks of the walk-in cooler, and floor tile and light fixture damage.
Spring Street Treats, located at 321 W. Spring St., was inspected on July 27 and received zero critical and one non-critical violations. The inspector found customer drink cups stored on the floor of the service shed.
Subway, located at 17 E. High St., was inspected on July 27 and received zero critical and six non-critical violations. The inspector found no drying devices near the hand-washing station, a utensil scoop stored in direct contact with food, damage to the cove base of the kitchen, mold on the mop sink, employee personal belongings not stored away properly, poor lighting by the cooler and walk-in freezer.
SOHI Grilled Sandwiches, located at 17 E. High St., was inspected on July 28 and received three critical and four non-critical violations. The inspector found a raw carton of liquid eggs over ready to eat foods like sliced cheese, the quaternary ammonium sanitizing solution at an incorrect balance, and refrigerated, ready-to-eat foods not properly date marked. For non-critical violations, inspectors found no soap at hand washing sinks, improper storing of wiping cloths, no food protection certified manager, and improperly stored utensils.
Buffalo Wild Wings, located at 10 E. Walnut St., was inspected on July 28 and received zero critical and three non-critical violations. Inspectors found an improperly sealed backdoor, a dish machine not fixed to the wall, and damaged floor tiles.
Bruno’s Pizza, located at 31 E. High St., was inspected Aug. 2 and received zero critical and four non-critical violations. Non-critical violations include cracked floor tiles, rusted cooler doors and cracked light fixtures in the walk-in cooler.
Skipper’s Pub, located at 121 E. High St., was inspected Aug. 3 and received zero critical and six non-critical violations, They were for a gap between cellar doors; food storage under waste water lines; dirt build-up on cooler and freezer doors; unnecessary equipment; dirt build-up on walls and ceilings of the kitchen and wall, ceiling and floor damage throughout the restaurant.
The Marcum Hotel & Conference Center, located at 351 N. Fisher Drive., was inspected on Aug. 4 and received zero critical and five non-critical violations. These non-critical violations include the use of a residential microwave in the hotel lobby, ice buildup on a freezer door, build-up on a cooler door, a damaged cabinet door and dust on fans in the walk-in cooler.
Patterson’s Cafe, located at 103 W. Spring St., was inspected on Aug. 4 and received two critical and four non-critical violations. Both critical violations for using a dirty can opener and storing food not at an improper temperature were corrected during the inspection. The non-critical violations include old stickers and residue left on food pans, damaged handles on kitchen utensils, a damaged door on the freezer, unnecessary equipment kept in the facility and an open can of soda being kept with clean dishes.
Oxford’s Kroger, located at 300 S. Locust St., was inspected on Aug. 4 and received one critical and nine non-critical violations. The critical violation, for not having a document related to employee health policy, was corrected during the inspection. The non-critical violations included not having an employee certified in food protection, using unlabeled food containers, an unsealed door near online grocery storage, a damaged door in the milk walk-in cooler, dirt buildup on equipment, a broken dish machine, ice buildup in several walk-in freezers, cracked light coverings throughout the facility and dusty fan guards in walk-in coolers and freezers.
Bodega, located at 38 W. High St., was inspected on Aug. 4 and received zero critical violations and five non-critical violations, which included not having a sanitizer test kit, dirt build-up on a non-food contact countertop, wall damage, a missing cove base behind the stove and dust build-up on the fan guard in the walk-in cooler.
Kofenya, located at 38 W. High St., was inspected Aug. 7 and received four non-critical violations.
Comfort Inn, located at 5056 College Corner Pike, was inspected Aug. 8 and received four non-critical violations.
Don’s Carry Out, located at 4325 Hamilton Richmond Road, was also inspected Aug. 8, and received two critical and eight non-critical violations. The critical violations were corrected during the inspection and dealt with no record of workers signing an employee health policy, and a damaged Hobart deli slicer product holder with the teeth ground off in portions of the product holder.
El Burrito Loco, located at 102 Locust St., was also inspected Aug. 8, and received three non-critical violations.
La Rosa’s, located at 21 Lynn Ave., was also inspected Aug. 8 and received one critical and six non-critical violations. The critical violation was corrected during inspection and dealt with cooked pasta in the walk-in cooler with no prep date marked.
Oxford Aquatic Center Concessions, located at 701 Kay Rench Drive, was also inspected Aug. 8 and received one critical and two non-critical violations. The critical violation was corrected during inspection and dealt with not having a record of workers signing an employee health policy.
Yum Yum Chinese, located at 24 E. Park Place, was also inspected Aug. 8 and received eight non-critical violations.
Gaslight Brewhouse, located at 325 S. College Ave., was inspected Aug. 10 and received six non-critical violations.
Jimmy Johns, located at 23 E. High St., was also inspected Aug. 10 and received six non-critical violations.
KDS Express, located at 4985 College Corner Pike, was a also inspected Aug. 10 and received five non-critical violations
Krishna Carry Out, located at 28 W. High St., was also inspected Aug. 10 and received five non-critical violations
Left Field Tavern, located at 12 W. Park Place, was also inspected Aug. 10 and received two critical and four non-critical violations. The first critical violation was corrected during inspection and dealt with a can opener having built-up grime. The second critical violation dealt with the presence of flies and gnats in the facility’s kitchen.
Miamian, located at 114 W. Walnut St., was also inspected Aug. 10 and received five critical and eight non-critical violations. The critical violations, corrected during the inspection, were for not having a record of workers signing an employee health policy, leaving soiled rags in the basin of the hand sink, leaving cream cheese mixture with no date marking in the walk-in cooler, and having a dirty hand sink. Several of the violations were repeats.
The Knolls of Oxford main shop, located at 6727 Contreras Road, was inspected Aug. 14 and received two critical and three non-critical violations. The critical violations pertained to a build-up of grime on the facility’s can opener and holding milk cartons and single-serve whipped butter at 8-11 degrees higher than regulation. Both violations were corrected during the inspection.
Oxford Sushi King, 37 E. High St., was also inspected Aug. 14 and received five critical and six non-critical violations. The critical violations involved dumping pitchers and cups into the handwashing sink, blocking its use; leaving a steel wool sponge in the basin of the hand sink; build-up of mold-like substance on the inside of the ice machine; and beansprouts, crab meat, and fish foods found in flip top prep cooler at 46 degrees, four degrees above regulation.
Doughby’s, located at 36 W. High St., was inspected Aug. 15 and received one critical and four non-critical violations. The critical violation involved not having a record of employees signing the health policy.
Brick House Cafe, located at 11 W. Church St., was also inspected Aug. 15 and received three critical and nine non-critical violations. The critical violations included raw beef stored over ready to eat foods and raw shell eggs over fruits and vegetables, and applesauce, yum yum sauce, and spicy mayo that were prepped over 24 hours prior to inspection and stored in the walk-in cooler without a label.
Bell Tower Dining Commons, located at 401 E. High St., was inspected Aug. 18 for a pre-licensing inspection before it opened for the semester. The restaurant received no violations.
Pickle and Pig, located at 77 S. Main St., was inspected Aug. 21 and received one critical violation and six non-critical violations. For the critical violation, food was not being held at the proper temperature and had to be thrown out. The restaurant corrected the violation during the inspection.
Miami University’s Clinical Health Cafe, located at 421 S. Campus Ave., also was inspected Aug. 21 for its 30-day inspection. The cafe received no violations.
PKPP Eta Upsilon, located at 110 S. Campus Ave., was inspected Aug. 22 and received one non-critical violation. The violation was a repeat offense for not having light bulbs covered. The inspector wrote that PKPP Eta Upsilon would be given another inspection during the school year when it returned to normal working hours.
O’Pub, located at 10 W. Park Place, was also inspected Aug. 22. The restaurant received six non-critical violations, one of which was a repeat offense of the facility lacking an employee with manager certification in food protection. The inspector also noted that the business was not open to the public at the time of inspection.
Corner Grill and Drinkery, located at 12 E. Park Place, was also inspected Aug. 22 and received one critical and three non-critical violations. The critical violation was for having unclean equipment food-contact surfaces and utensils.
The same day, the inspector also went to Alpha Delta Phi, located at 22 S. Campus Ave., but gave no violations because it was not open for business yet.
Delta Upsilon, located at 400 E. Vine St., was inspected Aug. 28 and received two critical and two non-critical violations. Critical violations included not informing employees about health policies and incorrect chlorine concentration in the dishwasher. Other violations included a damaged cooler and an uncertified non-commercial microwave.
Indian Ridge Golf Club, located at 2600 Oxford Millville Road, was inspected Aug. 28 and received two critical and four non-critical violations. Critical violations included grime buildup on the ice machine and incorrect chlorine concentration in the dishwasher. Other violations included missing hand-drying towels at sinks, incorrect storage of equipment and utensils, infrequent cleaning of nonfood surfaces, and cracked floor tiles in the kitchen area.
Johnny’s Campus Deli, located at 209 E. Sycamore St., was inspected Aug. 28 and received one critical and four non-critical violations. The critical violation, which was corrected during inspection, was not informing employees about their health policies and responsibility to report illnesses. Other violations included certifying managers in food safety, using an uncertified non-commercial microwave in the kitchen, having damaged or unused equipment in the kitchen space, and cracked floor and ceiling tiles in the facility.
Beta Theta Pi Fraternity, located at 200 E. High St., was inspected on Aug. 25 and received one critical and one non-critical violation. The critical violation involved damaged kitchen utensils and surfaces that could not easily be cleaned, which were both corrected during the inspection. The non-critical violation involved food items being stored directly on the floor.
Church Street Social, located at 107 E. Church St., was inspected on Aug. 25 and received three critical and eight non-critical violations. The critical violations included inaccessible hand-washing sinks, a lack of cleaning chemicals in a dishwashing sink, and improper storage of toxic chemicals, all of which were corrected during the inspection. Other violations included manager certification, use of soiled wiping cloths, improper storage of clean equipment and utensils, use of an uncertified non-commercial blender, use of damaged drink coolers, infrequent cleaning of walls and dish racks, and improper lighting. An employee drink was also left on a prep surface, and this was corrected during the inspection.
Delta Kappa Epsilon Kappa Chapter House, located at 325 E. Sycamore St., was inspected on Aug. 25 and received two non-critical violations. Non-critical violations included flaking and peeling paint on the hood above a stove, and dust and food debris on the ceiling near the hood system.
Yum Cha, located at 411 S. Locust St., was inspected on Aug. 25 and received no violations.
Sigma Alpha Mu Fraternity, located at 206 S. Campus Ave., was also inspected on Aug. 29 and received five critical and eight non-critical violations. The critical violations addressed the improper storage of toxic materials because bleach was found next to the prep sink. The can opener had food and grime build up, employees didn’t wash their hands, there wasn’t a health policy signed by employees, and the person in charge was unable to demonstrate knowledge about food safety.
Phi Gamma Delta Fraternity, located at 130 E. High St., was inspected on Aug. 29 and received two critical and four non-critical violations. The critical violations dealt with a lack of information telling employees about their responsibility to report health information, and food not being properly separated or stored to prevent contamination.
Oxford Lanes, located at 4340 Oxford Reily Road, was inspected on Aug. 29 and received one critical and three non-critical violations. The critical violations were for having unclean food surfaces and utensils. The ice machine had grime and mold built up on it.
Asia Market, located at 5930 Fairfield St., was inspected on Aug. 29 and received zero critical and 10 non-critical violations.
Poplar Asian Cuisine, located at 20 S. Poplar St., was inspected on Aug. 31 and received one critical and six non-critical violations. The critical violation involved food surfaces not being easy to clean. The deli meat slicer was damaged and worn down.
